Good Faith Estimate (No Surprises Act)

Providers are legally required to provide clients with an estimate of the expected charges for services when they do not have insurance or when they are not using insurance for said services. All clients at Challenged Hearts Healing Center, LLC have the right to receive a Good Faith Estimate which provides an estimate of what your mental health care will cost. This estimate is provided before you begin services and explains the expected cost of services include session costs, late cancellation fees, no show fees, as well as additional services fees. If you receive a bill which is at least $400 more than what is documented in your Good Faith Estimate you can dispute the charge. Challenged Hearts Healing Center, LLC reserves the right to change rates at any time, all clients of Challenged Hearts Healing Center, LLC will be updated directly and provided a new Good Faith Estimate to reflect any future rate changes. Challenged Hearts Healing Center, LLC reserves the right to refuse payment types such as cash, check, zelle, venmo, cash app, paypal, money transfer, money wire for example. All clients of Challenged Hearts Healing Center, LLC will need to have a working card on file for all payments to be processed.

Challenged Hearts Healing Center, LLC charges the following for full fee services:

  • 15-minute consultation call: Free

  • 53-minute individual therapy session: $210

  • 53-minute family therapy session: $210

  • 106-minute individual therapy session: $420

  • 159-minute – 424-minute individual EMDR intensive: $630-$1680

  • No-show or late cancellations (sessions canceled without 48 business hours of notice): $210

  • Sessions that exceed 53-minutes are subject to a fee of: $52.50 per 15 minutes.

  • Phone calls longer than 15-minutes: $52.50 per 15 minutes, the first 15 minutes are free.

  • Letters: $105 per letter

  • Form completion/Administrative tasks Specially Requested: $210 per hour ($52.50 per 15 minutes)

  • Additional or specialized services will have their own unique estimates and will be provided separately if and when relevant, as well as estimates for sliding scale.

Example Estimate:

If we follow the most common structure — meeting weekly for a 53-minute psychotherapy session at $210 per session — the following estimate for 12 months of psychotherapy services applies:

$210 × 4 sessions/month × 12 months = $10,080 per year

​

If we meet more or less frequently, or engage in activities other than just a “standard” therapy session once per week, your total cost will vary. You will never be charged for services not rendered outside of incidents that unfold in accordance with the late cancellation/rescheduling policy. Folks who pay on a sliding scale will have a good faith estimate which reflects that pricing shift.

All clients of Challenged Hearts Healing Center, LLC should always refer to their individual bill to determine the cost they owe as well as their own Good Faith Estimate as well as all applicable law for further conditions on payment and rights thereto. If they have questions, they should contact Challenged Hearts Healing Center, LLC directly and not rely on this page to answer them.
